Mine came with the vest, headrest, tether and crotch strap to keep him from kicking at the back seat window. You might want to look at the tether to be sure it will fit in your vehicle. It fits perfectly in a sedan with two top tether hooks in the back seat. My hefty son is about 80 lbs and I got the XL which should fit him for a year or more. The company can make custom sizing on the vest and tethers if you're needing something specific. I wouldn't travel without this!
